Mainstem intubation

Mainstem intubation: this occurs when the endotracheal tube is advanced beyond the carina, more common on the right than left

Decreased or absent breath sounds on the non-ventilated side

Chest x-ray with collapsed lung (can potentially be confused for massive hemothorax)

Tip: endotracheal tube tip deviates to the right, mediastinum shifts toward collapsed side (left) on this x-ray

Retract endotracheal tube back until breath sounds audible on the left and recheck chest x-ray
